Our experienced RNs provide round-the-clock medical supervision and care coordination. They manage medication administration, monitor health conditions, and serve as primary liaisons with healthcare providers. Our nurses are trained in both medical care and behavioral health interventions

Our specialized mental health nurse brings expertise in psychiatric nursing and dual diagnosis treatment. They provide crisis intervention, therapeutic support, and specialized mental health assessments while coordinating with our medical team for comprehensive care.

Our QM Coordinator Melissa ensures that all services meet the highest standards of care and regulatory compliance. They monitor service quality, coordinate training programs, manage documentation systems, and implement continuous improvement initiatives to enhance resident outcomes

Our Home Health Aides round out our interdisciplinary team whom meet regularly to review resident progress, adjust treatment plans, and ensure coordinated care delivery. We maintain open communication with external providers, families, and community resources to support each resident's success

My sister Maria is Special Needs and has struggled mentally for nearly a decade. My family tried everything to help her: multiple hospitalizations, treatment programs…nothing worked. She needed long-term support and we were exhausted. When we found Shalom Homes, we were cautiously hopeful because of the many previous disappointments.
From our first visit, we could see this place was different. The staff actually listened to our concerns and included our family in her treatment planning. They included kept us informed about Maria's progress and helped us understand how we could best support her. The team approach really worked - everyone from the nurses to the doctor communicated with each other and with us.
Maria developed a job skill, she loves flowers and now works at a floral shop. I’ve never seen her this happy and full of energy. The staff celebrated every small victory and supported her through setbacks. She still lives there but it’s like she more independent, working full-time, and we all have our sister back, it’s like we can all breathe again. We're forever grateful to the team at shalom Homes.
